Personal Information

Do you self-identify as a person with disabilities?
This question is optional, but we seek an accurate count for the training grant application. We do not require any details about your disability. Note that The Americans with Disabilities Act (ADA) defines an individual with a disability as a person with a physical or mental impairment that substantially limits one or more major life activities.

Predoctoral Research

The NIH requires that we submit information on students’ research experiences before entering our graduate program (for example, during and after completing your undergraduate degree).

Please estimate how many hours of research experience you completed before entering MIT (for example, as an undergraduate or as a technician). Do not include labs associated with a course (e.g. organic chemistry course with lab).
